Just a quick post today.
I was lucky enough to present at the Cerner Conference last fall. Aspects of that discussion, and some further work with some vendors, added up to become this article:
I want to memorialize that work here with the acknowledgment that a lot of talented people have helped to make this concept a reality. It is still early days, but we are learning every day about what this type of transition truly means in the evolving care delivery model.
